Creatinine Clearance Calculator

Estimated glomerular filtration rate via the Chronic Kidney Dysfunction-Epidemiology Collaboration 2009 equation (eGFRCKD-EPI) replaces creatinine clearance (CrCl) via the Cockcroft Gault equation as the preferred method of estimating kidney function in cancer patients. In some clinical situations it may be necessary to use directly measured glomerular filtration rate (mGFR) (e.g. radioisotope measured GFR). This follows recommendations from the International Consensus Guideline for Anticancer Drug Dosing in Kidney Dysfunction (ADDIKD). eviQ have developed an Estimated Glomerular Filtration Rate (eGFR) Calculator to assist in this calculation.

CrCl mL/min (males) = (140 - age) x weight(kg) / 0.814 x serum creatinine (micromol/L), for females, multiply result by 0.85

  • The calculator should only be used for patients older than 18 years.
  • The calculator does not accurately predict creatinine clearance in patients who have unstable renal function (e.g. patients in intensive care, patients with acute renal impairment, patients with febrile neutropenia); a measured (urinary) creatinine clearance is most accurate in this situation3.
  • The Cockcroft-Gault equation is unreliable in extremes of body size (that is, in severe malnutrition or obesity) therefore the calculator should not be used in these patient populations.
  • In obesity, actual body weight should be used in the calculation of CrCl via the Cockcroft-Gault method to obtain more reliable measures of renal function.2 However, it is worth noting that the original formula was only tested in a non-obese patient population.1
  • Ideal body weight for men2 = 50 kg + 0.9 kg for each cm over 152 cm
  • Ideal body weight for women2 = 45.5 kg + 0.9 kg for each cm over 152 cm
  • Adjusted body weight 3 = IBW + 0.4 (actual body weight - IBW)
