Care at home after an extravasation injury - cold or warm pack

 The following is a guide for methods of management at home:

Drugs that are vesicants and require require hyaluronidase and warm compress - vinblastine, vincristine, vindesine, vinflunine, vinorelbine.

Drugs that are vesicants and require DMSO 99% solution and cold compress - amsacrine, dactinomycin (actinomycin D), daunorubicin, doxorubicin, epirubicin, idarubicin, mitomycin.

Drugs that are vesicants and require cold compress - cisplatin ( > 0.5 mg/mL).

Drugs that are irritants with vesicant properties and require cold compress - bendamustine, docetaxel, doxorubicin liposomal, melphalan, mitozantrone, paclitaxel*, nab-paclitaxel.

Drugs that are irritants with vesicant properties and do not require any compress - oxaliplatin.

* There have been some clinical reports of positive outcomes with using hyaluronidase as an antidote for paclitaxel extravasation. If hyaluronidase is administered, no compress is recommended.
